Fair Contracting for Health Care Practitioners Survey
Description
The Pennsylvania Health Care Cost Containment Council (PHC4) has been directed by the legislature to study the effects of limiting non-compete agreements for certain health care practitioners in Pennsylvania.
This survey collects email addresses and NPI number or license number. PHC4 will use this information to identify and track survey responses. PHC4 will never release any identifiable information or individual responses that could be connected back to a single person. Further, PHC4 will not use the email addresses collected in this survey for any purposes other than the verification of survey data.
PHC4 plans to distribute an accompanying survey for a second round of data collection during early 2027, results will be compared over time.
Survey
In consideration of your time, we offer two survey options:
- The basic survey contains 9 questions and takes <4 min to complete
- The expanded survey asks an additional 10 questions and takes <14 min to complete
This survey asks for email address and NPI number or license number to verify respondents are health care practitioners. Should we have questions about your responses, we will reach out via email. All survey responses will be anonymized.

The Fair Contracting for Health Care Practitioners Survey will open on February 15, 2025 and close on April 15, 2025. During that time, health care practitioners will be able to complete the survey directly from this webpage. Please check back then.
